掲題の件に関して、2025年度も藤沢事業所構内において下記の通り集団接種を実施致します。

インフルエンザ予防接種は任意接種ですが、ご自身の状況に合わせて集団接種等をご検討頂き、

積極的にお申込みを頂きます様宜しくお願い致します。

１．集団接種日時　　１０月２８日（火）９時～１２時、１３時～１７時

２．場所　　　　　　１号ビル１階　ブリーフィングルーム

３．対象者　　　　　WDTJ/WDMJ藤沢事業所 構内勤務者

４．接種費用　　　（１）WDTJ健康保険組合加入の方

　　　　　　　　　　　 自己負担無料　＊全額健保で負担致します

　　　　　　　　　（２）上記以外の構内勤務者

　　　　　　　　　　　3,500円/名(税込)　＊当日に現金にて徴収します。

５．その他　　　　　１０月初旬に申込み要領等の詳細通知を掲載致します。

インフルエンザ予防接種を受けるには以下の方法があります。

１．藤沢事業所構内の集団接種で接種する

２．任意の医療機関で接種する

集団接種	勤務時間中に接種が可能 ヘルスポイントの補助申請が不要	接種日時・場所が限定
任意の 医療機関	都合の良い日時で接種が可能 被扶養者と一緒の接種が可能	場合により休暇取得が必要 後日、ヘルスポイントの申請が必要

Influenza Prevention Group Vaccination in FY2025 (advance notice)

 

In fiscal year 2025, an influenza group vaccination will be held at WDTJ's Fujisawa Site.

Influenza vaccinations are voluntary, but we ask that you consider the group vaccinations, etc., depending on your own circumstances,

We ask that you consider and proactively apply for the group vaccination, etc., according to your own circumstances.

 

 

Notes: 1.Vaccination dates and times: October 28 (Tuesday), 9:00 - 12:00 and 13:00 - 17:00.

2.Place：Western Digital Technologies  Fujisawa 　Building No. 1 1st floor Briefing Room

3.Target:Employees working on the Fujisawa Site

4.Vaccination cost

 (1) For those who are members of WDTJ Health Insurance Association

　　　　　　　No co-payment required   *Full cost will be borne by the WDTJ Health Insurance Association

(2) Other

　　　　　 　　3,500 yen/person (including tax)
