2022/09/12 Influenza Vaccination Point Subsidy for FY2022 will begin!

 We will start accepting applications for the FY2022 Influenza Vaccination Point Subsidy on Monday, October 3, 2022.

Please note the following points when applying for the subsidy and receiving the vaccination.

When applying

Please submit an electronic application or a paper application (by mail).

In addition, since many receipts are found to be incomplete every year, please make sure that the following items are written on the receipt before submitting your application.  Please make sure that the following items are included on the receipt before submitting your application. If any of the following items are missing, please present the receipt to the medical institution and ask them to fill out the form.

Receipt Required Information

(1)Name of inoculated person (if more than one person has been inoculated, the full names of all inoculated persons)

(2)Date of vaccination

(3) Cost (unit price)

(4) Proviso: "Influenza vaccination fee" or "Influenza" should be written on the receipt.

If the information is written on the medical statement instead of the receipt,

please attach the medical statement together.

(5) Name of medical institution

(6) Seal of the medical institution or person in charge

If you are submitting electronically, please upload the receipt (and medical statement,
if necessary).No separate mailing is required.
