at the Fujisawa site premises this year as well.

４．Vaccination ｔime and expense

 （1）WDTJ Health Insurance Subscribers

①Vaccination hours: 9:00 a.m. to 12:00 p.m. or 1:00 p.m. to 5:00 p.m.

＊Please select the time you wish to receive the vaccination and apply for it.

②Expense：Members have no personal burden

 (all fees are covered by the health insurance association)

(2) Other

　　　① Vaccination time: 9:00-10:00 or 16:00-17:00

　　　② Expense: 3,500 yen (tax included)

*Collection will be made in cash at the reception desk on the day of the vaccination and a receipt

will be issued.

５．How to Apply

(1) WDTJ Health Insurance Subscribers

　　　　Please apply through the Health Insurance Website "Health Examination Reservation System".　　　　　　　　　　　　　　　＊If you need to change the time or cancel your reservation after it has been made, please contact

　　 　　＊Please call the Briefing Room (51-2981) directly if you need to cancel on the day of the event.

 (1) Please fill out the attached medical questionnaire in advance and do not forget to bring it with you
on the day.

　＊Please take your body temperature at home or at the entrance of the office and fill in the
medical questionnaire in advance.

②Proof of ID (with your CAS or name, etc.)

(2) WDTJ Health Insurance Health Point subsidy is available for those who are not eligible for the group vaccination and for their dependent family members who receive the vaccination at a voluntary medical institution.
