Change of Name
If the insured person changes his or her name due to marriage or divorce, certain procedures must be followed.

Please notify your company immediately, attach a Health Insurance Eligibility Certificate (only if issued) to the application form and submit it. For those who have a Health Insurance Eligibility Certificate, the health insurance society will issue new Health Insurance Eligibility Certificate with your new name based on information from your employer. * A notification to the Western Digital Technologies Health Insurance Association is required even if an insured person or dependent continues to use their maiden name at their company.
* There is no need to attach your Individual Number Card.
