Death

The Western Digital Technologies Health Insurance Association pays funeral expenses (family member burial expenses) if an insured person or dependent dies. Please make sure to complete the procedures for the loss of health insurance eligibility.

50,000 yen

Death of an Insured Person

The Western Digital Technologies Health Insurance Association pays a uniform 50,000 yen for the funeral expenses in the event an insured person dies and is buried by family.
If the insured person does not have any family, the person who handled the actual burial receives burial costs up to 50,000 yen as funeral expenses.

Death of a Dependent

The Western Digital Technologies Health Insurance Association pays a uniform 50,000 yen for family funeral expenses in the event a dependent dies.

Procedures

Please fill in all of the required items on the Payment of Funeral Fees (Expenses) and submit it together with all of the attached documents necessary.
(Refer to the “Application Forms” page for information on where to submit application forms.)

Also, please follow the procedure below.
Death of an Insured Person
  • Documents proving that the insured person has died
    （Employer certification, a copy of the death certificate, a copy of the burial permit, a copy of the cremation permit, etc.）
  • Return the Health Insurance Card or the Eligibility Confiemation Document If he/she has it (notification through employer)

*Items to be attached if any of the following apply

【Funeral Expenses：If the claimant is not a dependent of the Western Digital Technologies Health Insurance Association】
  • Documents whose livelihood was maintained by the insured person（Certificate of residence etc. Documents listing the deceased insured person and claimant.）
【Funeral Fees：When the person who actually performed the burial makes a request】
  • Receipt of expenses required for burial（Original to the claimant's name）
Death of a Dependent
  • Documents proving that the dependent has died
    （Employer certification, a copy of the death certificate, a copy of the burial permit, a copy of the cremation permit, etc.）
  • Attach the Health Insurance Card or the Eligibility Confirmation Document if the person has it to the Dependent Change Notification (Removal)
