Lost Health Insurance Card or Eligibility Confiemation Document

Insured persons and dependents need to handle their health insurance card or Eligibility Confirmation Document with care because the Western Digital Technologies Health Insurance Association cannot freeze them even if lost or stolen.
If your health insurance card or Eligibility Confirmation Document is lost or stolen, you must immediately report the loss to the police and complete the lost health insurance card or Eligibility Confirmation Document procedure with the Health Insurance Society.

After December 2, 2024, a health insurance card will no longer be issued, please register your Individual Number Card as a health insurance card or apply to issue an Eligibility Confirmation Document.

Insured persons with optional continued insurance should contact the Western Digital Technologies Health Insurance Association.

After submitting Application for Reissuance of health insurance card, Eligibility Confirmation Document or Elderly Beneficiary health insurance card Due to Loss/Damage, please take one of the following actions:
Registering your Individual Number Card as a health insurance card Please refer to "How to Register your Individual Number Card as a health insurance card" under "Use of an Individual Number Card as a health insurance card".

Please take one of the following:
Registering your Individual Number Card as a health insurance card Please refer to "How to Register your Individual Number Card as a health insurance card" under "Use of an Individual Number Card as a health insurance card".
If you do not have an Individual Number Insurance Card and would like to continue to use your Eligibility Confirmation Document to receive medical treatment, please apply for a reissuance of your Eligibility Confirmation Document. Loat Eligibility Notification

If you wish to have your Eligibility Notification reissued, please submit an Application for (Re)issuance of Eligibility Notification.
If you have a smartphone, you can check your health insurance eligibility information (health insurance code and number, etc.) from the Individual Number Portal eligibility information screen.

Lost Individual Number Insurance Card
