Lost Health Insurance Card

Procedures

Insured persons and dependents need to handle their health insurance card with care because the Western Digital Technologies Health Insurance Association cannot freeze the health insurance card even if lost or stolen. Please immediately notify the police in the event of a lost or stolen health insurance card.
Please also make sure to complete the procedures to reissue a health insurance card. Insured persons with optional and continued insurance should contact the Western Digital Technologies Health Insurance Association.

Procedures

Please immediately submit an Application for Reissue of Health Insurance Card, Elderly Recipient Certificate, and Eligibility Certificate for Ceiling-Amount Application if the health insurance card has been lost or damaged.
